REGISTRATION PROCEDURES

Poster (Individual/Team)

1. Read the provided guideline book.

2. Complete the registration form in the DELI SCIENCE NURSING 2026 REGISTRATION FORM (https://bit.ly/formulir-pendaftaran).

3. Complete the Originality Declaration Form.

4. Complete the registration Google Form according to the specified requirements.

5. Upload the completed registration form and Originality Declaration Form to the designated Google Form.

6. Send your work and other required documents (as specified in the Guideline) to dsciencefkepusu@gmail.com.

7. Confirm your registration with the competition contact person listed in the Guideline.

8. Join the participant group for your chosen competition category (the group link will be provided after the registration form has been completed).

Video Health Campaign (Individual)

1. Read the provided guideline book.

Note: If you are participating in the Mobile Legends competition, enter your Mobile Legends ID and the corresponding account name of each team member in the designated section. Please enter each member's ID carefully to avoid errors.

Note: Participants in other competitions do not need to complete this form.

Please download the registration form and Originality Declaration Form using the links below. Complete all fields in capital letters and ensure that all information is accurate and complies with the specified requirements:

Steps for Downloading Documents from Google Docs to Microsoft Word

1. Open the document link.

2. Click “File” in the menu.

3. Select “Download” and choose the Microsoft Word (.docx) file format.

Note:

1. Participants are advised to print the Registration Form and Originality Declaration Form first. After completing both documents, upload them to the Google Form in PDF format and ensure that the file size complies with the specified MB limit.

2. Make sure that the email address and telephone number you provide are active. Attach all supporting documents according to the requirements in the guideline book and send them to the official organizing committee email address: dsciencefkepusu@gmail.com before the deadline specified in the competition requirements.
